Once you have scheduled an appointment you will receive an intake packet. (Be sure to check spam folder if you do not see it). This will come to you via the email you register with. Once you complete this intake packet your appointment will be confirmed. Make sure to complete these forms as soon as possible. If forms are not completed, including consents and history information, we will be unable to see you and may need to cancel your appointment.
